This full-time, hybrid remote position is located at the SRHD main campus, has a dedicated Monday-Friday dayshift schedule, and starts at $31.71/hr.

What you'll be doing

The Locating out of Care Specialist (LOOCS) provides complex low-barrier case management to PLWH who are lost to care. LOOCS conducts intensive field-based outreach to clients. This position works to improve and maintain access and adherence with clients needing extra professional support to stay engaged in HIV care. This position provides data and monitoring of care plans, viral loads, medical visits, and adherence to help achieve optimal health outcomes for PLWH. LOOCS collaborates with medical case managers, program managers, DPH, DOC, HIV providers, PEERS, community partners, and internal HIV prevention employees. LOOCS tasks are conducted with little supervision and provide low-barrier case management to the most complex clients, requiring the most advanced professional interpretation of policy and procedures. This position engages in clinical quality management work.
The company may provide initial client transportation to services. Client transportation is only authorized with the use of a company vehicle.

Some Essential Functions Include:

  • Collaborates with Medical Case Management (MCM)DOH, and medical providers to locate and re-link People Living With HIV (PLWH) who are receiving MCM services and have fallen out of medical care
  • Performs complex client intakes, eligibility, comprehensive assessments, releases of information, and individual service plans for out-of-care individuals
  • Coordinates out-of-care clients' access to primary medical care and treatment. Attends client medical appointments to ensure engagement
  • Conducts data review and observation of systems and processes to identify potential risk areas for falling out of care and best practices

What we need from you

Required Qualifications:

  • Bachelor's degree in social or biological sciences, counseling or other closely related field required. Master's degree is preferred
  • Three years of experience in social services, case management, or counseling
  • A valid motor vehicle operator's license and current auto insurance are required to complete the essential functions of the position
An equivalent combination of education, experience, and certification may be considered, provided the individual's background demonstrates the knowledge, skills, and abilities required for the position.
